Lipocine Inc. has announced the initiation of the BLOOM Phase 3 clinical study for its investigational drug candidate LPCN 1154, also referred to as oral brexanolone, to treat postpartum depression (PPD). The study is designed to evaluate the efficacy and safety of a 48-hour, at-home treatment regimen for the condition.

The BLOOM study is a placebo-controlled outpatient trial expected to enroll approximately 120 participants with severe PPD. The company anticipates the first participant dosing will occur in early Q4 2026. The study aims to generate prospective evidence to support a potential New Drug Application under the 505(b)(2) pathway.

The protocol incorporates feedback from the U.S. Food and Drug Administration (FDA) and measures to address findings from the company's prior Phase 3 trial. In that earlier trial, the primary endpoint was not met in the full analysis set, but exploratory analyses suggested rapid improvement in depressive symptoms when data from one high-enrolling site was excluded. That site had issues with the absence of detectable study drug in blood samples for a portion of participants.

To mitigate these risks, the BLOOM study includes strengthened site and data-quality controls. These measures include third-party support for site selection, geographic diversity, and the exclusion of sites with significant operational or data quality concerns. The study also includes prespecified sensitivity analyses to address potential outlier sites and pharmacokinetic data.

Lipocine reports that as of June 30, 2026, the company had $23.3 million in unrestricted cash, cash equivalents, and marketable investment securities. The company expects its existing resources to be sufficient to execute the BLOOM study, with an average cash use of approximately $1 million per month during study execution. The company notes that these expectations are dependent on study duration, enrollment, costs, and other operating requirements and do not imply that existing resources will fund regulatory approval or commercialization.

Lipocine’s pipeline also includes candidates for major depressive disorder, epilepsy, essential tremor, obesity management, liver cirrhosis, and preterm birth prevention.
